Give the full property address on the form. The City and the Town are separate governments with separate code offices and permit processes, and a mailing city doesn’t settle it.
The City’s code information states that roofing and siding require a building permit. We confirm the requirement for your specific scope against the address.
It changes the coordination, not the work: who is told when, access to the property, and what condition it’s left in each day. Say so on the form so it’s planned for rather than discovered.
Its approximate age, and what’s painted. Paid work disturbing painted surfaces in pre-1978 buildings may bring EPA Renovation, Repair and Painting requirements into scope. If you suspect asbestos-containing material, say so and have a qualified professional evaluate it before anything disturbs it.
